MSFM: Infectious Diseases, Psychiatry, and Pulmonary Medicine

Clinical questions to be addressed:

  • Infectious Diseases
    1. What is the role of serum procalcitonin in the management of lower respiratory tract infections? Does it reduce the use of unnecessary antibiotics?
    2. What is the efficacy of the H1N1 vaccine in mitigating symptoms and/or limiting pandemic spread?
    3. What are evidence-based methods for the prevention of refractory Clostridium difficile colitis? When and how should testing be performed for Clostridium difficile antibiotic sensitivity?
  • Psychiatry
    1. What psychiatric screens are recommended for the evaluation of Medically Unexplained Symptoms? What treatment modalities and followup are most effective in these patients?
    2. When should medication trials for depression be augmented and when should they be switched?
    3. How should treatment of chronic pain differ in a patient with a history of addiction?
  • Pulmonary
    1. How does the general internist evaluate the efficacy of continuous positive airway pressure for patients with obstructive sleep apnea? How is treatment failure defined?
    2. In the treatment of chronic obstructive pulmonary disease and asthma, are there evidence-based differences between the various inhalant delivery vehicles (e.g., dry powder inhalers vs. metered-dose inhalers)?
    3. When should phosphodiesterase type 5 (PDE5) inhibitors (e.g., sildenafil) and/or endothelin-receptor antagonists (e.g., bosentan) be used for patients with pulmonary hypertension?
