MSFM: Diabetes, Women's Health, and Anticoagulation

Clinical questions to be addressed:

  • Diabetes
    1. For the diagnosis of diabetes, how does the HbA1C compare with previously used tests? In which clinical situations should the HbA1C test not be used to diagnose diabetes?
    2. What is the optimal glycemic (HbA1C) target for type II diabetic patients?
    3. What is the risk for cancer associated with glargine? Should this risk modify our use of this medication? How is detemir similar or different?
  • Women's Health
    1. What is the evidence-based algorithm for evaluating dysfunctional uterine bleeding? What steps should occur prior to referral to a gynecologist?
    2. How are oral contraceptives (OCPs) used to bridge perimenopausal symptoms? What evidence exists for risks and/or benefits of using OCPs for this indication?
    3. How should comorbid conditions, such as a family history of breast cancer or a personal history of migraine with aura, be considered in the decision to use OCPs? In which clinical settings are OCPs absolutely contraindicated?
  • Anticoagulation
    1. What are the evidence-based guidelines for the use of newer anticoagulants, such as fondaparinux, argatroban, bivalirudin, and others?
    2. How long should patients with venous thromboembolism be treated with anticoagulants, and which patients are better managed with extended treatment with low-molecular-weight heparin (LMWH) rather than warfarin? How should patients on extended treatment LMWH be managed?
    3. How will the prevention and treatment of venous or arterial thrombosis change with the introduction of new, rapidly-acting, oral thrombin or factor Xa inhibitors?
