Multiple Small Feedings of the Mind: Endocrinology, Nephrology, and Lipids

Clinical questions to be addressed:

Endocrinology
1. How should we screen for vitamin D deficiency? Specifically, which test at what interval and should this be seasonal? How is adequate treatment response determined?
2. Should patients on bisphosphonates be given a drug holiday? How long is a holiday when treatment preparations vary by weeks to years? What diagnostic labs or tests should be followed during a holiday?
3. When should testosterone be measured in aging patients? What evidence is there that testosterone replacement is beneficial?
Nephrology
1. In type II diabetics with microalbuminuria, should an ACEI or an ARB be first line? When should both be used? When should a renin inhibitor be used?
2. When should acetylcysteine for outpatient CT and MRI scanning be utilized? Which patients are at risk for gadolinium-induced nephrotoxicity?
3. When should a patient be worked up for recurrent nephrolithiasis? Which tests should be initiated by the internist?
Lipids
1. What is the evidence that treatment of hyperTG/low HDL influences CV outcomes?
2. Discuss the ENHANCE and SEAS data on exetimide and the current prescribing recommendations.
3. What are the preparations and diets available for omega fatty acids supplementation, and what kind of a benefit is expected?
