Perioperative Medicine
1. Which patients should be treated with stress dose steroids perioperatively? How might patients on chronic steroid therapy be treated differently from those with a history of steroid use?
2. Can medical therapy with beta-blockers and rigid heart rate control obviate the need for noninvasive testing in intermediate risk surgical patients?
3. How do you manage antiplatelet therapy perioperatively in a patient with a cardiac stent?
Osteoporosis
1. How significant is the risk offor jaw osteonecrosis in patients treated with bisphosphonates? Should the dental care of patients on bisphosphonates be altered?
2. What is the optimal duration of therapy with bisphosphonates? What does research specify about drug holidays?
3. When is it indicated to measure vitamin D levels? If deficiency is identified, how should it be replaced?
Headache
1. Are the newer prophylactic drugs for migraine, e.g., topiramate, better than the older treatments such as betablockers and tricyclic compounds?
2. How would you identify medication overuse or analgesic rebound headache? What is the best treatment strategy?
3. When should oral contraceptives be used or avoided for treatment of hormonal migraine headaches?
