Diabetes and Insulin Resistance
1. Where do inhaled insulin and DPP4 inhibitors fit in the management of diabetes?
2. In light of recent data regarding serious adverse cardiac effects, fluid retention, and increased risk for fracture, when should a diabetic patient be continued or started on a TZD?
3. What are appropriate lipid targets for a diabetic patient? Is an LDL <100 low enough? When should high triglycerides be treated?
Psychiatry
1. placing a depressed patient on an SSRI antidepressant, how should the dose be titrated? What type of monitoring should be instituted, particularly with regard to suicidal ideation?
2. How do you recognize and treat PTSD? When is psychiatric referral necessary?
3. What modalities are effective and safe for long-term treatment of chronic insomnia?
Treatment of Menopausal Symptoms
1. What is the safety profile and effectiveness of topical/vaginal estrogens for vaginal dryness and atrophy?
2. What is the difference in efficacy of bioidenticals/compounded hormones and FDA approved prescription HRT preparations?
3. How is the cardiovascular effect of HRT different in perimenopausal vs. postmenopausal women? What are the implications for treatment decisions?
